How to Submit Health Benefits Enrollment Forms for Postdoctoral Scholar Benefits (Click HERE for Instructions)

  • Log in to UCPath online

  • From the home page, select Ask UCPath

  • On the menu bar, select My Inquiries or Submit an Inquiry.

  • From the inquiry page select or type:

    1. Topic: Select "Benefits"

    2. Category: Select "Postdoctoral Scholar Benefits”

    3. Subject: Select "Postdoctoral Scholar Benefit Enrollment Form"

    4. Description: Type "Submitting enrollment form to make life event (OR) newly eligible elections”

  • Add Attachment (Applicable form(s) must be uploaded)

The combination of the subject line, topic, and category outlined above will assist in proper routing of cases and easy identification of the request.

NOTE: If you are submitting the enrollment forms on behalf of the employee, please use the same topic/category and subject line to ensure that your case is properly routed.

Requests within a PIE require one of the following enrollment forms. Please ensure the form is complete, signed, and submitted within the PIE period.

  • Postdoctoral Scholar Health Benefits Enrollment Form for Newly Eligible Employees

  • Postdoctoral Scholar Health Benefits Enrollment Form for Life Event

Requests outside the PIE must also include the Postdoctoral Scholar Late Enrollment Request. All sections of the form must be completed and the reason for the late enrollment request should be explained in detail for it to be considered for exception processing.
